Widget slide not advancing on training retake. Captivate 2017.

Captivate 2017: I've created a course that ends with a quiz. The learner may fail the quiz two times before being directed to retake the entire course. During testing, I intentionally failed the quiz twice and used the "Retake Training" button I created on the quiz results screen to start the course over from the first slide. Everything works fine until I get to the 6th slide, which is an interactive accordion widget that asks the learner to view each of the 3 tabs in the widget (screenshot below). I have a "next" button navigation control set up on the slide, and I also have the "Forward" button on the playbar enabled. Neither of these buttons will work in this situation. This issue only occurs when I've failed the quiz twice and then retook the training. It does not occur when I fail the quiz once then retake the training. I've replicated this issue both on my published project in our LMS as well as in the SCORM cloud. Any ideas what could be causing it? I've also included a screenshot of the SCORM cloud results - no idea if it's helpful since I'm not really sure how to interpret the results anyway. Any help will be very much appreciated!!

UPDATE: I've been trying different things, like deleting the widget slide but the issue still occurs at that same spot. What I didn't mention is that there is a knowledge check slide right after it, so it seems like that slide is the culprit. I tried changing the knowledge check slide from 2 attempts to infinite, but that didn't work either. Hopefully this helps shed some light?

What are the Quiz settings? Especially the field 'required'?

It must have something to do with the fact that I have the quiz set to two attempts. The issue occurs only when I've failed the quiz twice, then go to retake the training; not when I've failed the quiz once then retake the training. So I guess I have a new question: how can I set up the module so that the learner has two attempts to take the quiz before being forced to retake the training, then having another two attempts at the quiz? Perhaps I should just keep it at two attempts, but then instead of allowing them to restart the training within the module, set it up in the LMS so that they have to be reassigned the course?

Do not use that option, set it back to Optional. There are other ways to force the user to answer all. Take out playbar, don't allow Skip.

If you have only 2 attempts on Quiz level, the Retake button should disappear after the second failure on quiz level. I don't know what is wrong there.
