problems with forms at www.medanswering.com

www.medansering.com is the website for Medical Answering Serice, that provides transportation for Medicaid doctor appointments.  If the patient is driving himself, he is reimbursed for the expense; and there is a form to fill out.  This form - along with a score of other forms - is downloadable at the website.  Most of the forms print out easily, and so did the third in the list, the Physician Attestation Mileage Reimbursement Form.  That is until yesterday, when I tried to print it and started getting the message:

I've tried numerous Acrobe Reader updates in two different browsers, and nothing helps.  All the other forms except the fourth in the list print, as do my other .pdf files.  I'm using Reader 10.1.2, and there are no available updates. The phone number of Medical Answering is 315-475-1601.

In the link that you are using (http://www.adobe.com/support/reader/), you will see a pop up window open when a customer support person is available for chat. You can work with customer support to resolve  your problem.
