FrameMaker 12 to Microsoft HTML Help?

We are trying to create OnLine Help (chm) files from FrameMaker 12. I've figured out some of the mapping, but one whole area has me stumped! It is how to get a hierarchical Table of Contents in the OnLine Help like this:

Also, as shown above, we'd like the "Title" on each page to not appear in the TOC!

I couldn't find any samples that resulted in the type of thing I'm looking for.

Any help would be much appreciated!

    What does your FM TOC look like? Since the "publishing engine" inside FM12 is really RH, the conversion of FM TOCs to RH ToCs is governed by the indents in the FM TOC.
