Black set to 100%K but not holding on separations

I have tried everything I can think of. It's only happening in the stroke behind the top text. I have made sure fill and outline colors are set to C 0%, M 0%, Y 0%, K 100%. I have expanded, released compound path... nothing is working. All black separates out as 100%K except the top. Can anyone tell me what's going on?

(Tried to upload the ai file, but it keeps getting rejected... file doesn't match it's extension type???? So I'm uploading the PDF)

I'll ask a dumb question: what are your settings in the Preferences dialog box for Appearance of Black? For printing exporting is the setting "Output All Blacks Accurately" or "Output All Blacks as Rich Black"?

The text does not have a stroke, it has a filled outlined text behind it that is fill with a CMYK mix.

Cmd or Ctrl click twice on the red TAXPAYER text to select the object below it.

Select menu > Same > Fill Color  to select this mixed filled objects.

Fill them with black.

File has been modified...I removed all the layers at the top except for the text, then I changed text to outline, and created a stroke around the text. The stroke was set for 100%K (all other colors are are 0%).  The stroke still will not stay with 100% K. It shows when I select that layer in the layers panel that it is filled with 100%K, but when you do a separations preview, it is still there when you turn off all black.  I am still unable to upload an AI file.

JUst change the name of the file from .ai to .pdf

What do you mean with "not holding"?

Also: please change the file extension to .pdf, but the file should be an AI file, not a PDF.
