Duplicate links when packaging

Using CS2015 and OS 10.12.6/Sierra. I packaged an Illustrator file that contained one psd link placed four times in the file. Instead of collecting the one link. It duplicated the link 3 times so that I had a total of 4 psds in the Links folder. It also renamed them. So, I started out with Art.psd and I ended up with a Links folder that contained; Art.psd, Art_1.psd, Art_2.psd, Art_3.psd. Has this issue come up before?

You can update to latest version of illustrator and try if you are getting the issue or not.

Yes, that was tried and did not resolve the issue. It seems to be server related. Thank you for your reply.
