I am absolutely livid right now. I sent my manufacturer an updated PDF file of our final packaging design. Turns out that our QR codes are missing and instead printed as black squares. In illustrator, the image says "Embedded" in the top left corner. When I hover over the word Embedded a tooltip pops out and says "No file associated with this image." How the hell does this even happen? When I open a previous version of the same artwork file, the QR images are there.

In a PDF file, images are always embedded.
So I'm wondering exactly how that manufacturer printed that PDF. Maybe they didn't use Acrobat, but tried and import it in some third party application?

When I open v5-22.ai or v5-22.pdf the images are missing. Yet in v5-21.ai or v5-21.pdf they are there. I don't think it has anything to do with PDF, but more of when I saved the illustrator file as a new version (new filename).

You probably save the PDF with the option "Illustrator editability". In that case when you open the PDF inIllustrator, it opens the PDF part of the file, not the PDF.

Ignore the .PDF files for the moment.
The issue is I had a file v5-21.ai which had embedded QR code images.
When I saved the file to the next version v5-22.ai, the QR codes disappeared. I didn't realize this at the time (as it gave me no warning) and they're visually smaller on the overall larger artwork file.
In v5-22.ai these images say they are embedded, but they're not there and when I hover over the word embedded is when it gives me the "No file associated with this image" error. How does this even happen?

Which system is it?
Where are the files located?
Which file format?
Exactly with which options did you case the AI file?
Did you not check the PDF file before sending it off to the printer? You must then have noticed that the codes are not there.

Windows 10.
Both files are located in the same folder which is running Google drive (backup & sync, so they are permanent local copies).
As mentioned previously they are *.ai file format.
I do not change any options when saving to new versions in *.ai.
I didn't bother to look for the QR codes because they have always been there in previous versions. In addition they are small compared to the overall artwork. I have multiple embedded images in the document, yet only the 3 QR codes went missing while saving to a new version.
Clearly this forum is unhelpful. I will reach out to Adobe support.

PLease tell us the exact steps that led to the issue.
How did you place the images, how did you save the file? Which app are you using to check your PDFs.
WHich version of Illustrator is it?
