Hi-
I have a document I made in Illustrator. I have a few fields such as name, sig, and date that I would like to be fillable on a computer. Is this possible with AI on OSx and if so where can I find instructions on how to do it?

Daniel's answer is correct.
Of course, Illustrator and Acrobat Pro DON'T exist (yet) on OSx. They're available however on Windows and Mac operating systems.

Apple's names are confusing. I think for OSx we can read OS X, the approved name from 2012 of Mac OS X, and since 2016 officially called "macOS", which few of us have caught up with... meanwhile there is no Illustrator or Acrobat for iOS. Actually, there is an Acrobat for iOS, but it doesn't do this...

my bad, i should have just said Mac OS. of Mac or whatever. Or maybe it doesnt even matter. Sucks I have to get Adobe Pro to do this on top of Illustrator. Thx all.

If you have only one Illustrator file that you want to make into a fillable form, you could download the 7-day free trial version of Acrobat. But of course it expires in 7 days, so if you need to edit the form after that or create new forms, you'd need to buy the Acrobat software.
