My company's graphic designer sent over the CMYK/ Hex Colors she is using in Illustrator. When I enter the CMYK number I get a different hex # and when I enter the same hex # I get a different CMYK number. Both of our computers are set to CMYK color mode. If you compare the two blues, there is a slight difference. I want all of our documents to be consistent so I am matching her CMYK color but I do not understand why there is a difference.

Your different results are happening because color management is set up differently. But even then the values don't need to match at all, because maybe the designer determined a value not by conversion, but by looking at it.
Next time when posting photos please compress them better and/or make them smaller. Not everyone has broadband connection at all times. Your message took 5 minutes to load.

I agree with Monika, you probably have different document profiles.
Go to Edit...Color Settings, and then make sure you both have the same profile plugged into the CMYK field.
