Can anyone tell me why this happens when I save Illustrator files as PDFs? The image is a linked PSD and the green rectangle is behind the image but not sure why it looks like this in PDF format.

What standard are you using to create the PDF? Try using the PDF/X-4 format. This format maintains all transparency of the original images used to create it.

Tried to replicate that, but couldn't. Can you check for blend modes in the Transparency panel?
Or share the file?

The blend modes are normal. But it looks like the PDF/X-4 format is working. Thanks.
