Width tool splitting on path

Occassionally, when I use the width tool on a shape I've created with the curvature tool, it segments the path where it is still connected. I haven't been able to discern where in the process this has gotten messed up and how to fix (if it can be) afterward.

This often happens when there is a corner point. But that corner in the upper left could even be 2 points stacked onto each other.
