How to bend objects to path

Can illustrator be used to draw a 5-line music score that bends and warps and gets smaller in the distance, like this:

I would like to be able to use a photo of a music sheet, and then be able to bend it arbitrarily.

If there's no easy way to do it in illustrator, any guess how this kind of stock image is produced?

Hello,avpromedia

The put a beautiful curve as the sample is, I think very difficult.
But with Illustrator's 3D function, it is possible to some extent.

(Sorry for the Japanese version. Please guess.)

1. First, register the score as a symbol.

2. Draw a line.

3. "Effect"> "3D"> "Extrude & Bevel"  direction "top"

4. Paste the score symbol in the mapping. At this time, check "Do not display structure".

5. Press the OK button to close the 3D effect panel.

Add a curve to the path drawn in “2” using “Effect”> “Distort & Transform”> “Zig Zag”.

6. Select the 3D effect in the Appearance panel and rotate the object. Adjust “Perspective”.

Rotate the object. Adjust “Perspective”.

It could be as simple as a sufficiently high-resolution texture applied to a 3D object. Otherwise indeed in AI this can only be done the hard way with tons of envelope warps and similar or drawing it cleanly from scratch.
