Creating a submitable form in indesign

Hi, I am wanting to create an interactive PDF for a client that wants to send it to clients to try and sort out their own small IT issues that they don't have time for. This will be in the form of problems which will be linked to pages with possible solutions. Yet if the problem is not fixed after the solutions they are wanting a form in the same document that can be filled out and submitted via email.

I have attempted this using "mailto:" but it ends up sending the whole PDF via email.

Is there a way to send just the last page of the document that's been filled in rather than all of the information they already have? Any help would be appreciated !!

This is not something you’re going to accomplish with InDesign. Why not put an HTML form on your company intranet and use that?

Was just wondering if it was possible as a short-term solution as the companies website is very outdated and is going to need some work to get up and running but I may play around with creating a link to an HTML with the survey on and link the answers to the business email. Thank you!

A JavaScript could be added to the PDF via Acrobat Pro to send whatever parts of the PDF/Form you want.

If that might work for you, I suggest you post a question about doing this on the Acrobat Scripting forum. Folks there a phenomenally generous with their help and advice.

But the ability to add JavaScripts to a form in InDesign isn't available. I sure hope it will be added in the future!

Not natively, of course, but it can be done already with our FormMaker add-on for InDesign: FormMagic | Id-Extras.com
