The easiest way is to use Acrobat Pro to add an image field, so that the script is automatically integrated.

You can't add Acrobat scripts to form fields (including buttons) in InDesign natively.

It is possible with FormMaker (which is free for small, 1-page forms): https://www.id-extras.com/products/formmaker/
