InDesign Cross-References do not update

Next issue

Developing 500 page book and Cross referencing (CR) such as "see Sexual and reproductive health, page xxx".

This CR might need to be used dozens of times throughout the manual. I have 90 topics which also require CR as required.

 

 

Have made Text Anchors according to all the online forums and official Adobe instructions, however:

- when i insert or remove a page from the book the error "the referenced paragraph or text anchor is missing"

 

- when i select the whole book and "update all cross-references", cross-references do not update

 

- i have to update cross-references manually all over again

 

This is my 3rd edition over 8 years of this manual and i dont understand why this is happening

Any help appreciated

please do not send links to "how to cross-reference" - been there done that
