indexing proper nouns, names - last name first

Hello, I am new, so please be patient. I am creating an index with proper names, which I want to list with last name first. (Smith, Tom) I realize I can use shift-option-command-bracket to get proper order, but I don't see how I can "add all" when I do that; it goes directly into index without giving me "add all" option. I also don't understand how I can edit name to "add all" once it's in index. Help? Thanks.
