Problem with quotes and apostrophes

I have a long document with ASCII quotation marks and apostrophes throughout where there should be Unicode left/right single and double quotation marks. Three questions:

1) How did this happen?

2) How do I prevent it from happening again?

3) How do I fix it?

Hi Marie,

you are asking how this could happen:

I assume you assigned the language [No Language] while importing.

Either you synched to a paragraph style in your InDesign document that has [No Language] applied as value for language or you selected text with [No Language] during import or if you pasted from Word.

Before doing anything to fix it, first let me know, if this could be the issue.

Later we could fix it with a couple of GREP Find/Change actions.
