Shifting Text in a PDF during export

We placed a client provided PDF in an .indd file (CS6) and the font at the top appeared fine.  We then exported a print file and noticed after the project was finalized/printed that the top lines shifted up and down.  We took a closer look at the client provided PDF and noticed each text line was duplicated on top of each other.  The client wants a technical explanation of why their text shifted as such in print.  Anyone dealt with this before and know how this happens?

How did you make your PDF?

Why is the imagery of the PDF in CMYK, it should be in RGB for InDesign. Ask the client to provide you a PDF/X-4 for placing in InDesign, Images should remain in RGB.

We can certainly ask the client for this in the future, but the current proof has already printed - our print file will always be CMYK so it would be converted anyways.  We place all types of provided PDFs into our InDesign files, some in CMYK, and some in RBG and this has never happened before.  They just want an answer as to what caused this issue.

THe font is twice in the PDF, why it is shifted I don't know. If you inform us on hwo you did YOUR PDF, we would find the reason.

This PDF was originally set up in RGB and converted to CMYK, that is visible on the black elements, not professional done.
