Creating an index with multiple references

I'm in the middle of attempting to create an index that contains thousands of names (medical abstracts). An index was provided to me in a Word doc, and is structured like this:
Last Name, First Name Co-author or Author Page # Title of abstract
My issue is how to have the names reference the abstract title within the book, while keeping the page numbers associated also intact (or if that's even possible).
Currently, all of the names have been indexed, but I keep thinking there must be some magical way to tie each name to it's title (the title is it's own unique paragraph style). I would LOVE to avoid having to manually input names with their abstract titles, as I may have a few birthdays in the process.

what about creating TOC? As you have the Paragraph styles for the naves and titles, you can set the TOC levels to them, and the program will add the page numbers.

Unfortunately that only does half the job, with either capturing the title or the names, not both.

Actually, I think I misunderstood. Wouldn't the TOC then list the title, and then the group of names underneath? I would need them all extracted to show as the first post.

TOC shows you any paragraphs which have their own Paragraph style. So you can extract all the records as you need. For a moment I'm not ready to say whether you can build them in one line or they will group under each other, but if you will need, I can check it a bit later. at least? you can always create such structure:
Last Name, First Name
Co-author or Author
Title of abstract Page #
Or
Title of abstract
Last Name, First Name
Co-author or Author Page #
