InDesign 16.2.1 - index not listed correctly after converting IDML file to INDD

Firstly I convert INDD files to IDML because files need to be elaborated in Trados which support only IDML files.
After IDML files are modify I need to convert those in INDD again.
This passage it disassembles completely the layers of the index file, it seems to happen from the third level like in this example and the PNG file attached:
Chapter 5
5.9
1.1.1
1.1.1
1.1.2
1.1.2.1
1.1.2.2
Is there anything I can do about it?

was the index created before or after the IDML file came back from the Trados system?
What's your version of InDesign?
What's your operating system?

Thanks for replying to me.
The index file was created before the returning of IDML files.
The Indesign Version that I'm using right now is the 16.2.1 and the SO is Windows 10 version 21H1.

I'd try to rebuild the index after you saved the IDML file from Trados as InDesign document with a new name.

then the IDML > Trados > IDML workflow wrecked your Index.
Are the index markers in the layout still on the right place?
Do you still have the IDML file untouched from the Trados system?
If yes, do you see the right order of index entries if you open that file as InDesign document?
If no, we have a bug with InDesign, if yes, there is a bug with the Trados system.

Hi Vladan,
We've also tried to convert IDML files in INDD even when they aren't passing through Trados, so only using Indesign.
And the problem still appears.

a similar problem was reported here but the version of InDesign was 2017.
