Does anybody know of a template or preset to print passport photos in Lightroom on a Mac?

When my daughter needed a passport photo I just created a template based on the requirements in the passport instructions. It's pretty simple to do. I don't have the template anymore because my computer crashed and I had to reinstall. But printer templates are not that difficult to create.
