3d extrusion seems to be cutting part of the object off

I am pretty familiar with 3d extrusions, but I have run into an issue that I cannot for the life of me figure out.  

Suddenly, when I extrude from a layer that has a round object, I am getting strange divots in the 3d extrusion.  What am I missing here???
