I recently installed CS6 suite on Mac (OS High Sierra 10.13.1), and when saving for web the titles that appear when hovering over options will not disappear until you quit the program completely. There are also some errors that make the layers appear incorrectly. Please see screenshot. Any ideas of what's going on?

Update your mac to High Sierra 10.13.5 and the problem with the tool tips should be alleviated.
Short of that you can go to Photoshop>Preferences>Interface and uncheck Show Tool Tips.
Also get the newest cs6 updates by going to Help>Updates in photoshop cs6.

If you use CS6 on High Sierra, you're skating on thin ice. That old version of Photoshop is not recommended for High Sierra and that may be the cause of the problem. Give consideration to upgrading to the CC version of the Photoshop application.
That said, have you tried saving the file (without Save for Web) as a sRGB 72 ppi psd and as a jpg, as well? What were the results?
Have you tried unchecking Preferences > Performance > Use Graphics Processor? What were the results
Have you tried flattening the file first?

Make sure you have all the Mac High Serria update install for you model Mac. You may need a newer Display device driver then is currently installed tips do not seem to be removed instead they may be being added to you document or may just be in your screen capture. They should be removed from your display after some time period.
