Custom Shape Problem

When I create a new custom shape (edit > "Define custom shape," in the past, it always showed up at the bottom of my custom shapes panel. Now they are going to a seemingly random folder. I need to search all my folders to find the newly created shape. What's up?

Which Group is selected in the Shapes Panel when you define the custom shape? 

Could you please post screenshots with the pertinent Panels (Toolbar, Layers, Shapes, Options Bar, …) visible?
