Getting a proper PNG icon

This image shows perfect in the following PSD file screen shot, but it gives a distorted PNG image when it placed on a folder in Windows Explorer. PNG icon is okey but it gets unproper image when it placed on a folder.

How to fix it?  PSD file is attached.

This is not a case if you use usual folder [folder given in Windows].
