Opening PDF Files at Size

When I use Photoshop to open certain PDFs I am not able to open them at size - I must input dimensions and dpi.  Is there a way to set Photoshop to open ALL files at size?

The first time you open any PDF into Photoshop the normal action is to require the size to be specified.

Do you mean dimensions? You can record action  to automatically scale after opening file. Action can be triggered using File > Scripts > Script Events Managder > Photoshop Event : Open Document.

The only time you won't be asked to select the size/resolution/etc. is when the PDF was saved from Photoshop, and it just gets back what was saved.

A PDF has to be 'ripped' (rasterized into pixels) for Photoshop so you have to tell it what you want when you open a PDF. Just like you would with a vector image from say Illustrator. They have no 'size' until you define how many pixels from non rasterized documents you want to be created.
