Photoshop shortcuts keep changing by itself every single time I restart the app.

I am using the photoshop cs6 version and recently I found out that my photoshop wasn't functioning properly. I am using it on my MacBook and now every time I restart the app, the control will switch again. I have used one of the previous posts in the community but it did not work for me.  

Even the setting is set to photoshop defaults, it still not going back where it should be. As you can see from the image above, the setting for quit photoshop is not the same as the one above. Is there a way to fix it?
