Ps - on save -will not default edited file to original name. Attempts to force name change.

Background: iMac Pro, MacOS 14.1.2 Sonoma. Ps 25.3.1, Lr 13.1 Canon .cr raw files.
Problem: This problem appeared in the transition to current versions of MacOS and Ps. Never occurred before. After file import from Lr and edited in Ps, upon save Ps ignores existing file name and forces change to a format of untitled-1234.psd. This can create a rogue file lost somewhere in the Lr catalog. Note. This happens only once after file is first imported. After that everything is normal.
Process: RAW files imported from card via Lr. Files are named upon import. File is selected and sent to Ps via cmd-e. Transfer successful. Editing successful. When I attempt to save/save as (in Ps) the program ignores existing name attempting to force a 4 digit + suffix format, e.g. untitled-1234.psd. I have to manually remember then correct the name before save. This can lead to distress when dealing with a large number of files on a deadline.
Discussion: I've researched this problem here and on photog forums. The only suggestion was to re-examine Ps settings in particular Preferences -> File save options -> then select, Enable legacy save as. This was already set and fiddling with it produced no behaviour change.
Request: I know the experts live here and would greatly appreciate assistance.
