Puppet Warp Tool Hides Important Parts of Image When Making Mockups

Hi everyone,
I attempted to make a mockup for a competition I was entering. It was partially an illustration from PSD and partly text added from Illustrator. I exported the image to a PNG. For my mockup I used a photo and placed the PNG above it, converted it to a smart object, and applied a layer mask.
First I transformed it to approximately fit the package, and then I applied puppet warp to it. When I applied the puppet warp, the left middle area of the image disappeared. When I turned off the smart filter, that area reappeared.
Why couldn't the puppet warp show all areas? Why did it ignore an area?
I had originally tried to create the mockup in Illustrator Beta, but the result was even less satisfying. Only parts of the text were shown, and the image disappeared altogether.
