Saved document opens as a new document

Hi everyone!  I teach a digital art class to middle school students and recently we have been having an issue with some saved documents.  The student will save the document ( as far as I can tell they did it correctly), but when we try to open it next class, it opens a totally new document.  It even opens the new document panel.  Any idea why this is happening or how to fix it?  I have had four students loose projects.  Thank you!

    If they open a totally different document of what is saved then someone has managed to overwrite the document, Photoshop cannot on his own display different document.

    As far as I know there isn't new document panel, do you mean new document window which opens on File > New? Is that happening after using File > Open and selecting file to open or immediately after opening file?

    Yes. I do.  It happens no matter which way I try to open the file.  I have done both.

    If the file opens, but is empty, then it's either overwritten by somebody, or corrupted by bad hardware.

    If double-clicking an existing file brings up a new, blank document, then Photoshop's preferences are corrupt and a reset should fix it. But the chances that this should happen on four different machines (or even four accounts on the same machine) is zero.

    There are many ways to open a file. Please describe exactly how the file is opened.
