I downloaded photoshop CS6 from creative cloud to my mac. Under filter/sharpen I do not have shake reduction. Is this a setup problem or a app. how do I get this?

The current version of Smart Sharpen also has a motion blur option to help with camera movement.

CS6 is still available, but it is no longer being developed, it's functionality and abilities are effectively frozen... to put it mildly, it is obsolete.
The reason it is still available it that Adobe knows many people, for differing reasons, loath the idea of the new subscription "service" that is Creative Cloud.
So, if you want the latest functionality, such as the "Camera Shake" filter, you have to sign up for Creative Cloud (the subscription service formally known as Master Collection), or wait 'til next month and sign up for Photoshop CC, as Mylenium stated.
But don't get the idea that Adobe is "forcing" you to sign on for Creative Cloud... no no no no no... absolutely not... you do have a choice... the obsolete, buggy and balky CS6... or all the shiny new stuff in CC!!!
Yer pays yer money and takes yer choice!

Well, to be COMPLETELY accurate, it will be on June 17. At the moment it's the current release.
I think "buggy and balky" is probaby a bit of an over exaggeration too - I find Photoshop CS6 13.1.2 to be quite stable.
