Shift click brush does not make straight line

Shift clicking the brush is not creating a connected straight line. Only two dots. Holding shift still makes a straight line, so I know it is registering the shift key. 

 

Tried two different keyboards, two different mice, and resetting the preferences.

In the Brush Setting panel, in the Brush Tip Shape tab, make sure the Spacing box is checked.
