Tools not working right.

Recently some of the tools have stopped working right. When I change size of the brush tool for instance the hardness and opacity color inside the tool disappears. Sometimes changing size of the tool it leaves behind a ghost tool. I noticed also that when this happens the grahics adapter is no longer listed under the performance settings and I can't get it back unless I close PS and reopen it. Of course the tool actions return as well, but this is getting ridiculous trying to work on a photo. Anyone got an idea of how to fix this?
