Brush not tapering, how to fix?

Ive been having issues for a few months with my brush ends not tapering. I dont know how to fix this and its making it harder to use as i need the tapered edges for most works, and it makes it hard to use bigger px brushes as this problem is way more visible the bigger the brush size is.
