Pixels seem rectangular

  1. When I draw a square, same height and width according to photoshop, it comes out rectangular even though the pixel aspect ratio is set to square.

It would be better if you posted a screen capture not a Photo. However, from your Photo it looks like its a Windows issue for the aspect ratio of objects on your windows tasks bar are off as well as all Photoshop objects.

Adjust your monitor resolution in Windows Settings > Display. Your whole screen image is stretched, it has nothing to do with Photoshop.
