Photoshop is deleting my actions and replacing them with default ones.

I’ve been having this problem for since the last update. When I open Photoshop I get this messege. 

And when I click OK, and open photoshop, all the files on the welcome page are gone.

and My Actions are deleted and replaced with Default ones. This happened so many times, i lost count. I have been using these actions for a long time and never had an issue.
