Increasingly terrible generative fill results??

So no matter what prompt I do essentially all the options are awful. I am wondering what on earth is happening with Generative Fill. Generative Expand mostly serves its function as expected for outdoor scenes but it's still doing really weird stuff for anything that isn't a natural structure.

It was this bad when it first started, then it got better, and now... what on earth is going on?
