How do I save individually filled PDF forms into Dropbox?

Background: my organization needs a mobile PDF form that can be filled by individuals and easily saved to Dropbox after every use, and then refreshed for the next user. It would also be preferable if this could be done in an offline mode, as we will be using it in areas without WiFi.
