Multiple Choice Check Boxes on Fillable PDF Not Working

Hiya. 

So I made a fillable pdf in order to make a consult form for my business. I had planned to put it on my Samsung Galaxy A8 tablet on adobe reader so I could fill it out, save a copy and send it to my clients when I got home....


While I am able to check the multiple boxes available on my desktop, I am unable to transfer the same file to my adobe reader on the tablet and still be able to check the multiple options. It allows me to click only one option....

 

However, when I first turned on the tablet, It worked ONE time, and I can't figure out how to fix it so I can check those multiple boxes again. Any idea on what I can do?

    I hope you are doing well. I am sorry for your experience with Acrobat Reader Mobile.

     

    Is this issue with a specific document or with all the files? I tried to reproduce the same at my end but was not able to.

     

    Please share the below details:

    1. The application version of Acrobat Rearder mobile you are using;

    2. The application through h=which the PDF was created;

    3. The android version your tablet is running;

    4. A sample file so that we can check at our end too.
