Could you please share more details about the issue? When they say “add image,” do you mean: (A) Insert an image into an existing PDF, (B) Add a photo from the camera/gallery?

What is the version of the Acrobat Reader installed? Check it from the profile icon from the top right in the app > Settings > About Adobe Acrobat. Please ensure you have the latest build of the app installed. Check for any pending updates from the App Store, install the updates, restart the app, and the phone.

Confirm that your account includes PDF editing.
Adding an image to an existing PDF requires an Adobe Acrobat subscription; it is not included with the free viewing and commenting features. If you already subscribe but Acrobat asks you to upgrade, go to profile icon > Subscriptions/Manage subscriptions > Restore Purchases.

Photos permission on iOS: To insert an image, Acrobat needs access to Photos. Go to Settings > Acrobat > Photos > Full Access (and Settings > Acrobat > Camera, if you will use the camera). If access is denied or limited, the image picker fails.

Test the image format (HEIC): The iPhone 15 Pro Max saves photos in HEIC by default, and Adobe does not directly support HEIC. Test with a JPEG or PNG, or change the camera format in Settings > Camera > Formats > Most Compatible (this makes new photos save as JPEG). If it fails only with photos from the camera roll but works with a JPEG, the format is the cause.
