Part of form disapeared

Hi,

Hope you are doing well.

We assume that you have been trying to fill out this PDF form using Adobe Acrobat Reader mobile app.

What is the source of this form, are you filling it after saving locally on device?

Its missing details will be available after export or not, that can only be checked if we look into the form or when you export it. 

Reason: This is probably the nature of the form or the way its form fields have been prepared, that upon filling the details it disappears or not visible. 

Conclusion: There's nothing much that you can change (preferences or form filed properties) through Acrobat Reader Mobile app.

As a workaround we can suggest to download a fresh copy of the form and start filling it. Or for testing, try filling it on freeware desktop application Adobe Acrobat Reader DC.
