How does Acrobat Reader manage to have full access to the photo library on an iPhone 12 without any permissions to do so AND have the Settings app report that Acrobat has no access?

That is some clever yet nefarious-looking programming.

Then why did I just get a notification on my android asking to turn an image I got from WhatsApp into a PDF? Permissions are disabled in the settings and I haven't used acrobat in ages.

Since file access permission has been provided by you to Acrobat, we aim to enhance your experience by identifying potential document images and providing an easy way to convert them to PDFs. You can disable these notifications by turning off the ‘Downloads’ channel in your app notification settings.
