There are whole books about this subject. There are also numerous tutorials on how to create forms. The start page for this forum has links to some. The help for Acrobat also has some reference material on creating forms. Try press the F1 key or opening Full Help.

Acrobat is not a good document creation program, so I would not create a form using this product. It can add form fields to a PDF document. I would crate the context of the form in a program like MS Word, Apache OpenOffice.org, or Libre Office. MS Word cannot create form fields that can be used by Acrobat.

Both Apache OpenOffice.org and Libre Office can create form fields within a document and then Export to PDF and create form fields usable by Acrobat. You will need to use Acrobat to add actions to the form fields created.

MS Word docs can be converted to PDFs. Once a Word document has been you can use the Form Wizard to locate most fields and then use the tools in Prepare Forms to cleanup the result of the Wizard.
