Create a model release form in AE

Hello!
I'm taking an online class in Professional Practices in Photography, and we're creating model releases.
I could bang this out easily in Illy, but I'm super curious about creating it in Express and I think that would be very interesting to the class, but I can't figure out how to tell Express to create it? How would this work?

I had very good luck creating pictures of what I thought my friend's backyard swimming pool should look like but I dn't know how to create documents, it has to have these elements:

  • Your Name or Business Name
  • Heading of Document: “Model Release”
  • Shoot Date and Brief Description:
  • All Parameters discussed: i.e “for valuable consideration”
  • Photographer Signature Line:
  • Photographer Printed Name:
  • Signature Date:
  • Subject/Model Signature Line:
  • Subject/Model Printed Name
  • Subject/Model Birthdate:
  • Subject/Model Address:
  • Subject/Model E-mail: 
  • Witness Signature:
  • Witness Printed Name:
  • Witness Address or Email:
  • At least 2 Photographer Contact info: Phone/email/web address/address
    Thank you in advance, a couple of years ago I basically learned Character Animator from the forum, and made a couple of episodes of a cartoon on youtube 😉

From my own experiments with Firefly actual output of text is not yet something it excels at. producing images and text effects - absolutely, but whole documents like you want here - Firefly may well struggle. It's not something I've seen done.

 

You could try asking in the Firefly forum, in case its something one of the experts there has tried or has ideas on how to approach it.
