Adobe Acrobat Pro Forms

I have created a form using Adobe Acrobat Pro and have filled in the form fields. Why does the content of these fields disappear when I prepare to send the document for esignatures?

 

I have attached a screenshot of what the forms look like at the different stages and have also attached the original form.  This form does this every time I complete it.

    p.s. i don't think the adobe website, and forums in particular, are easy to navigate, so don't spend a lot of time searching that forum list. do your best and we'll move the post (like this one has already been moved) if it helps you get responses.

    Thank you for moving my post and apologies for posting it in the wrong place.  I still haven't had a reply to my question but hopefully someone will respond soon!
