Adobe Pro DC and Adobe Sign

I have several users with Adobe Pro DC subscription. Randomly one wil report that when they are trying to setup a form to get multiple signatures that the screen for Adobe sign is just blank. 

I tried looking through the support docs for a solution to no avail.  Any help here would be great.
