Adobe Sign app error on iPad

I'm trying to send for signature from the Adobe Sign iPad app. I'm using a template that works as designed when I access from the web. When I use the app (which I'd like to do because I want to use in person signature feature), I get the following error:

Transaction failed because of a server error: unable to complete request. Unknown form fields provided (name of field)

The form loads, but I get this error when signing. I get the error whether I initiate using "In Person Signature" or "Send for Signature". If I access the same template from a browser on the iPad, I do not see this error.

I am supposed to train a sales team tomorrow and the in person signature feature was a big driver of this implementation! Can anyone help?
