Adobe Sign Document Sent Is Blank

I have setup my document for signatures, ensured that each field is required, each field NOT READ ONLY and each field identified for sender or recipient.

I fill out the areas with fill and sign and send for signature after pressing save.

When the document arrives, it has the initial and sign areas for the signer, but my filled sections for prices and such are blank.

What am I doing wrong. As it is now, the people look like they are signing a blank contract

Help!
