Adobe Sign - Form Field show if another box is completed

Hi - I have the form below in Adobe Sign and Account # field and Balance of Pay fields are to appear when something is in Routing field.  I have tried everything and cannot get it to work.  HELP! 

What condition would you suggest be put in for each field?
