Adobe Sign forms help

Hi

I have created my template forms which I need to send as a pack of documents to be filled in by one recipient.

Every time I test this, I fill the information in the forms out and submit but when I receive the completed forms, the data keeps pulling through.

The result is, I end up with contact numbers from one form appearing in another form about health.

Please can somebody help me understand why this is happening and how I can sort the problem out.

    In your process it sounds like you end up combining mutliple documents. if these doucment has form field which have the same name, they become duplicates and will behave as you describe.

    to avoid this ensure to name your fields in each document in a unique way.

    for for example if you use 3 different document and all 3 have a name field, name each name field like so:

    doc1-name

    doc2-name

    doc3-name

    or whatever system makes sense to you as long as it results in unique names.

    also it can be handy in case the is a legitimate reason to duplicate values.
