Adobe Sign - how to get the sent copy that include added fields

When requesting a signature, the signature field and any related fields are added to the PDF before sending it. My question is, how can I obtain a copy of the document with the fields added? When I check the agreement, it appears without any of the added fields.

    Please let us know if you are referring to creating a PDF form in Acrobat and then trying to send it for signature. Or trying to save it as a template.   

    If creating the form in Acrobat, click on the three dots at the top left former in the prepare form window. Then select "Prepare form for e-signing".

    Hi,

    I want to send a PDF for e-signature. My process is to request the e-signature, add signer information, and insert the necessary fields and then send. However, I want to keep a copy of the document after adding the fields but before sending it. When I review the agreement, it appears as though the fields I added are missing. Please advise. Thanks.
